Walter Simmons

Age 58 · Male · MRN 48 117 902 · Patient since Jun 2023 · Psychiatry: Dr. Benally · Addiction medicine: Dr. Kowalski

Major depressive disorder, recurrent, severe (F33.2)Alcohol use disorder, in early remission (F10.21)

Progress note

Combined medication management and recovery check in. Walter is seven months sober and remains active in his community based recovery group, which he credits with keeping him accountable. Today he describes his mood as flat rather than low, says he has stopped most of his morning routine including the walks he had kept up since early sobriety, and reports sleeping nine to ten hours most nights without feeling rested.

Across his last three visits, Haven's readout shows mood falling and fatigue climbing, the steepest shift on his chart since intake. He has not volunteered the loss of his morning routine in either of the last two visits on his own; it came up today only after being asked directly.

Plan: increase sertraline from 100mg to 150mg given the mood trend, continue addiction medicine follow up at the standard interval. Given the fatigue and withdrawal from his own routine, a same week check in call was added rather than waiting a full month, and his recovery sponsor was looped in with his consent.
